Instructions for the On-Line Evaluation Program

1. The link to the program is

http://survey.rossmed.edu.dm/crseval.htm

2. Or, access the Ross Dominica website at http://www.rossmed.edu.dm/ and click on Student resources, click on Grades On-Line, and click on the survey link.

3. In the Log In window, type your User Name which is the First name and Last name with a space in between, such as Joe Jones.

4. In the Password window, type your Banner ID number including the @ sign and all the numbers such as @0000012

5. If the User Name and Password do not work, or you changed your password and do not remember it, click on the Forget Your Password box. Then type your Ross email address and the program will send you the correct information to your Ross email account. Type the information exactly as it appears in the email.

6. When the website appears, click on the Survey List on the top bar.

7. Click on the Take Now icon.

8. Enter all the required answers.

9. Click Save at the end of the survey to record your answers.

10. Log Out when finished.
